Helen's Hope Chest ID

Identification Cards will be used as a tracking system for clients of our foster care closet.

Once a foster family is referred to us by CPS, The Family Resource Center, emergency referral, or any other crisis agency, an appointment will be made for the foster family to visit Helen's Hope Chest.

An ID card will be issued at the time of visit with proof of Department of Economic Security (DES) or Case Management number. That number is then recorded on the ID card.

This card will be effective for one year from issuing date. The card can be used 4 times a year by season, and is hole punched at each visit.

Extra visits can be made by case manager referral in the event of an emergency situation.

The ID number is then used to track the family in our Online Data Management (ODM) system.
